Prepare for Your Visit
To prepare for your visit, we recommend bringing the following, if applicable:
Previous medical records: This could include your last doctor's note, recent consultation notes, the last 1-2 years of lab results, and any imaging studies or tests you believe might be relevant to your current health concerns.
By providing this information beforehand, we can get a better understanding of your medical background and ensure your first visit is informative and efficient.
Reschedule a Visit
If you need to reschedule your appointment, please provide us with at least 48 hours' notice. This allows us to offer the appointment slot to another patient in need.
