1. Physical Exam

Comprehensive medical examination performed by our certified physician

2. Vaccination Review

Checking of records and vaccination recommendations to meet USCIS requirements

  • As per the law, the required vaccines (subject to age) for the Immigration Physicals are the following:

    • Measles

    • Rubella

    • Polio

    • Tetanus and Diphtheria Toxoids

    • Pertussis

    • Haemophilus influenzae type B

    • Hepatitis B

    • COVID-19

    • Rotavirus

    • Haemophilus influenzae type b (Hib)

    • Any other vaccine-preventable diseases recommended by CDC

3. Blood Tests

The blood work requirements include (subject to age)

    • T.B. Gold test (in case it comes positive, then you will be required to perform an X-ray)

    • Syphilis (in case it comes positive, then additional confirmatory test to order)

    • Gonorrhea

What to Bring to Your Appointment

01. I-693 Form

Please complete the first two pages of the I-693 Form if you're not able to print we will provide the form in our office, but bring your Alien Registration Number (A-Number) if you have one.


02. Valid photo ID

Bring your passport, driver's license, or other government-issued ID (e.g. State I.D., Consular I.D., or School I.D.) Please make sure that they are current/valid.


03. Optional

  • Vaccination records (translated to English if necessary) 

  • Medical history documents (e.g. Immunization, TB treatment or Mental Health records), if any
